I have created step by step how to guides for filing the E-Rate Form 471
both for Category 1 and for Category 2. Remember that you must file
separate applications for Category 1 and Category 2 services.

the PDF file for either "FY2015 Form 471 Category 1 Guide" or "FY2015 Form
471 Category 2 Guide" from the Keeping on Track Section on the right side
of the screen.
