*The FY2014 filing window for the Form 471 closes at 11:59 PM EDT on March
26, 2014.*

All forms and associated Item 21 attachments must either be filed online or
received via mail by the close of the window.

Make sure that you are using the revised Form 471, either by filing it
online using the 'Create Form 471' tool available on the "Apply Online"
page on the USAC website (http://ext.sl.universalservice.org/menu.asp) or
by printing out a copy from the USAC website (
http://www.usac.org/sl/tools/forms/default.aspx) if you are choosing to
file on paper. Do not use any old paper versions of the Form 471 or your
application will be denied. Check with me if you have any questions about
completing the form.

If you filed a new Form 470 this year:

   - you cannot file the Form 471 before the Allowable Vendor
   Selection/Contract Date identified in your Form 470 Receipt Notification
   Letter
   - the Contract Award Date listed in Item 18 on the Form 471 must be on
   or after the Allowable Vendor Selection/Contract Date but before the date
   you certify the Form 471
