That's a great question Tom/Debra!
 
I think everyone is correct in what they've said, but rules are never
that simple :-)
 
To answer your 2nd question first, to my knowledge, penalties are deemed
to have happened at the stoppage of play, whether they happened earlier
(delayed call), happened at the instant of the call, or happened during
the stoppage.
 
Now on to your first question ...
 
I don't think play *ever* continues at 5 on 5.
 
Rule 4-2.e states, in part..
   "When one minor penalty is assessed to one player of each
   team at the same stoppage of play, these penalties will be
   served without substitution provided there are no other
   penalties in effect and visible on the clock."

* What happens when a player from each teams gets a double minor?
 
* What happens when 2 players from each team get a minor?
