Without quoting the rule, helmets are covered by 3-4-a and are required
as you say, including the statement ... must replace ... or go to the bench.
Immediate whistle if goalie's helmet or mask are off.
  Sticks are covered in 6-5-a and the rule is as you said, other than goalkeeper
may continue with the broken stick.  A broken stick is one that is "unfit for
normal play".  A chipped or cracked blade is not broken if such blade is
entirely covered with tape and there are no projecting points.
 
  John, it is time to put aside your "outside activites", get your striped
shirt out and get on the ice where you can do some good.  We need your
kind on the ice.   (;-)
