In my opinion, the most frequently called penalty is roughing. A close
second would probably be tripping. You get roughing penalties almost every
game, and most of the time, you've got two guys going in the box together,
which is why it gets more calls than tripping, where only one player is
penalized (most of the time).
 
The least called penalty would be some extremely obscure penalty -
something like illegal deliberate substitution. Or probably something that
is obscure and anochronistic. No example here off the top of my head,
though.
