The referee should not hold the puck in two hands. The puck should be held in
the non-whistle hand, in clear view of the players facing off.  The puck should
be dropped flat on the ice to prevent bouncing, with as little motion as
possible.
 
You are correct in that some linesman/referees are very strict and some are not
in determining their criteria for tossing players.  This can be confusing for
the players until they get to know the officials tendancies.
