The FY2016 Form 470 is available through USAC's new E-rate Productivity Center (EPC) Portal.

If you do not have EPC set up for your billed entity, you will need to contact USAC’s Client Services Bureau at 1 (888) 203-8100 to create the EPC Account Administrator account for your organization.

Entities that apply for any E-Rate not arranged through Networkmaine must file a Form 470 if they have an expiring contract or want to request E-Rate for new services for FY2016.

A billed entity cannot file a Form 470 without first setting up its EPC account.
