The Form 471 that is to be used for E-Rate applications for FY2014 includes a new Item 24. This item replaces the old Block 2.

If you are completing a paper version of the Form 471, make sure that the version of the form you are using displays "FCC Form 471 – December 2013" in the footer.

If you are filing online, the correct version of the form will automatically be provided via USAC's Apply Online page (http://www.sl.universalservice.org/menu.asp).
