There is just one week left to submit a paper version of the Form 470. Any Form 471 paper version must be received by USAC by February 19. If you are filing on paper, you will need to mail it as soon as possible in order for it to arrive by the deadline.

You must post a Form 470 for every service for which you are seeking E-Rate discounts except for services for which you have a multi-year contract that will not expire until after June 30, 2015.
Paper Form 470 Submission Deadline: Wednesday, February 19, 2014
February 19, 2014 is the deadline by which USAC must receive a Form 470 submitted on paper. If you do not have an electronic PIN to certify USAC forms online, then you can either (1) submit and certify a paper form or (2) submit a form online and then mail a paper certification. In either case, the form and the certification must be received by USAC by this deadline. After you submit a paper form and/or certification for the first time, USAC will mail you a PIN so that you can certify forms online in the future. It usually takes about two weeks to receive the PIN.
Online Form 470 Submission Deadline: Wednesday, February 26, 2014
February 26, 2014 is the last day to post a Form 470 to the USAC website (and if necessary issue an RFP) and certify it online with an electronic PIN.
*** Remember that you must wait 28 days after the Form 470 is posted or you issue any RFPs before you can complete your competitive bidding process, select a service provider, sign a contract, and submit a Form 471. The Form 471 must be posted before the filing window closes at 11:59 PM EDT on March 26, 2014. ***_____________________________________________
