I was wondering if anyone would mind sharing the current form you are using for media permission and online web2.0 access, more specifically for Middle School (7-8 graders).

For example in order for students to register for Edmodo, Prezi or Khan Academy usage, etc. do teachers send forms home for parent signatures to allow student registration? Is it one blanket form for online usage and media permission?

Do you give your middle school students email addresses?
If a couple parents refuse, how would this be handled?
