We have just sent the 479 paperwork to all libraries, private schools and
school districts involved in the MSLN2 Consortium application for Year 4.
Libraries will receive their forms via mail. School Districts and Private
Schools will receive their forms via email from the Department of Education,
and those emails will go to Superintendents and Headmasters. The letter and
form are included below and will also be posted on the E-Rate webpage. Tech
Coordinators may want to contact their administrators to make sure that the
forms are completed and returned on time.
The 479 form is used to certify compliance with the Children's Internet
Protection Act, CIPA (the filtering bill) as an MSLN2 Consortium member.
