$150/ticket set with a non-refundable $6 application/handling fee.  $606
was charged to my credit card the same day I applied for my 4 ticket
sets.  The web site states they use interest earned to offset the ticket
system costs.

I entered my number, but the system balked.  When I entered my address,
it recognized the account.  As I recall, it was presented as a 10 step
process, but I was completed easily in just a few minutes.

Applications (1-4 tix/household) must be completed by 1 June to get a
chance at the allotted 9,500 tickets.
