As an immigrant I can't really pin it down for you, but I think it was as
late as the 60's.  In another life I do a bluegrass radio program.  One of
the tunes I played for Remembrance Day was "Our Canadian Flag" by Wilf
Carter.  This World War II era song refered to the Canadian troops
fighting under the Union Jack which was a dominant part of the Canadian
flag [ensign] at the time.  The Maple Leaf is a later adoption.
 
Anyway, I can confirm that in the NHL games here only the Canadian anthem
is played unless a US team provides the opposition.
 
The last two season of Edmonton Ice (WHL) games used the same system.
Alas the Ice have moved to Kooteney.
