No (at least I don't think so). A coach can, however, obtain game
misconduct. From Section 4.b. of the rule book:

A game misconduct penatly involves the suspension of a player,
coach, manager, trainer or any other team personnel for the
balance of the game; however a substitute is permitted to replace
      a player immediately.

Note: This is not to be confused with the disqualification penalty
plus a major penalty. A player receiving a game misconduct
penalty is suspended for the remainder of that game only. The
player is allowed to play in the team's next scheduled game.

>If so, why did the refs tolerate this idiot rather than toss him out?

   Good question.
