I thingk "5-hole" comes from the game "Score-O" (and related
beanbag-tossing type games at carnivals), where there's a 4' by 6' wooden
board in front of the goal, often with a picture of a goaltender painted
on it, with five holes --- 1, 2, 3, and 4 are the upper left, upper right,
lower left, and lower right corners, respectively, and 5 is in the middle
at the bottom (between the goalie's legs if there's a goalie picture
on the board). The object of the game, naturally, is to put the puck
(or beanbag or tennis ball or whatever) through one of the 5 holes.
