That all depends upon what you mean by "region".  If you mean simply "east" or
"west", it's been a fairly common occurence in the 90s: 1990 (BU, BC, Colgate),
1991 (BU, Maine, Clarkson), 1993 (Michigan, Michigan State, Lake Superior,
Wisconsin), and 1997 (Michigan, Colorado College, North Dakota).   This was
also the case at least once in the 80s (1986; I don't have a source right handy
to check for others).  If you mean more narrowly than that, 1993, with three
Michigan teams, is the only one that I can think of.  I'm pretty sure that's
the only time that one conference has put three teams into the final four.
