Probably the reason no one's ever seen it is that goalies (this side of Ron
Hextall, anyway) hardly ever get misconducts called on them.  But if this is a
rule change, it's not a recent one.  Back in 1990, Clarkson fans received
probably their first introduction to fan-favorite ref Harry Ammian when he hit
goalie John Fletcher with a misconduct when Fletcher argued for a crease
violation call after a goal was scored.  Fletcher had to serve the misconduct
himself, and his backup proceeded to lose the game.  I would guess the only
reason I remember this so well (Cornell was not the opponent) was that it was
the only time I had heard of this situation until now.
 
I don't have a copy of the rule book, but I think there is a section in there
about allowing penalty-box substitutions when a goalie is called for a minor or
major, but not for a misconduct.
