The way it has been explained to me is during hard collisions, the jaws come
together with a great deal of force. This in turn causes the concussion.  The
mouthpiece acts as a cushion of sorts in softening this collisions between the
upper and lower jaw.  I'm sure Tari will be able to better explain the medical
reasons on how this causes the concussion.
