Both Mike and Rick Scero have referred to the mouthpiece rule as 1st Offense,
removal from ice, warning. 2nd offense, Misconduct, 3rd Game Misc.  However,
I've since checked a *Hockey USA* article about the NC$$ changes this year and
they quoted the rule as "For the initial violation, the offending player will
be warned, sent to the players' bench and replaced on the ice.  For a second
offense by the same player in the same game, a game misconduct shall be
assessed."
 
So which is correct?  Two strikes and you're out or three?  Two strikes matches
up with what happened in a Metro Boston Hockey League (NC$$ rules) game at
the 1980 (14 year old) level.  One of the best players on a team -- a loose
cannon who tries to stretch the rules at times -- got tossed on the *second*
offense in a game.
