The blame in this case goes to the minor official who was running the box
and the penalty timekeeper.  No penalty is imposed, because the fault is
with the minor official (Now if the player jumps on disregarding the
minor official's warning not to, then you get a "too many men on the
ice").  When we have situations like this in our youth games, we always
go over the release rule with the person who will be letting the
player(s) out of the box.
 
A1   7:30 2 min
A2   7:00 2 min
A3   6:30 2min
 
A1 can come out at any whistle between 5:30 and 5:00.  At 5:00, when A2's
penalty expires, Team A is now entitled to 4 players on the ice.  If A1
is still in the box, he is released.  The same applies to A2.  A3 may
stay in the box for a while, depending on what is happening on the ice.
