Ed, I don't know why the concept has developed as it has, but this is
consistent with other all-tourney groupings I've seen.  Apparently, the MVP is
deemed to be anointed to "Super Hero" status, not to be associated with, or
mentioned in the same breath as, the mere "Heroes" named to the all-tourney
team.  It's not that he didn't make it; he's above it.  For example, if the
MVP is a goalie, the second-best goalie will be named to the "All-Tourney
Team"
