A couple of answers to a couple of rules questions...
 
Jeffrey Anbinder:

Rule 4-9-b(2), in part:
 
"If the penalty or penalties to be imposed are minor penalties (while
the teams are at equal strength) and a goal is scored on the play by
the nonoffending team, the first minor penalty shall not be imposed.
However, all other minor, major or disqualification penalty(s) shall
be imposed in the normal manner, regardless of whether a goal is
scored.  If any other fouls are committed on the same play or after
the referee has stopped play, the offending players shall be penalized."

Rule 6-7-a, in part:
 
"Note: A cross-check is defined as a check delivered by extending the
arms with both hands on the stick and making contact with the opponent
above the waist."

You're right, I can't find anything in the rule book to cover this,
either.  I don't have any idea.  At first I'd say the referee would award
the win to the other team, but then what if the team that doesn't have
enough players is winning and wants to give it a go - why not?

1-0, as far as I know.  The rule book only covers the situation where
a team refuses to obey the decision of the referee (6-38) - "...the
penalty shall be a forfeiture and the score shall be 1-0..."  I would
guess this also covers all cases of forfeits.
