Actually, the NHL at least used to have such a penalty; Dave Forbes of Boston
received one for spearing Henry Boucha in the eye in 1974.  The fact that the
incident occurred indicates that deterrence doesn't always work.
 
These matters really have to be handled on a case-by-case basis.  There
is just no way to anticipate everything.
