Rule 6, Section 1.b. says
 
   After a penalty has been called, a misconduct penalty shall be
   assessed to any player who challenges or disputes the ruling of any
   referee or fails to go directly and immediately to the penalty bench.
   If the player persists in such a challenge or dispute, the individual
   shall be assessed a game misconduct penalty.
 
Rule 6, Section 28.a. says, in part
 
   Except at the end of each period, or expiration of a penalty, no
   player may leave the penalty bench at any time.
 
   Note: Penalized players must remain on the penalty bench during
   stoppages of play and timeouts.
 
    (portions deleted about end of game)
 
Rule 6, Section 28.b. says
 
   A penalized player who leaves the penalty bench before the penalty
   has expired, whether play is in progress or not, shall incur an
   additional minor penalty, after serving the unexpired penalty.
