I suspect it happens fairly often in the early stages of the season, when some
teams start playing earlier than others.  Sometimes you'll see a preseason #1
lose a game or two, resulting in some other ranked team which hasn't even
played a game yet getting elevated to the top spot.
 
I think you don't usually see idle teams moving up to #1 this late in the
season... mostly because there aren't that many teams that have weekends off
this late in the season.
