I received this too - yes, it's for real. I wondered the same but it
looked pretty authentic and since it had a .gov address, I assumed it
was. I then checked with my insurance broker and they confirmed other
clients had received this recently too. I had to gather some
additional information from my broker to help complete it, but I'm in
the process of completing it now.
The slightly confusing part is that once you create a registration,
you have to wait a couple of days before going back and having the
survey be 'ready' for you to fill it out.
Then it will auto-populate with some of your staff (in my case, 1
current staff member and 1 former). It doesn't seem to need info on
all my staff but asks questions about whether the two individuals were
employed in 2008, and a few other minor questions. It is a multi-part
survey and luckily, you can complete part of it, save and come back to
it (as you will need to do as you gather data). For instance, I had to
gather the # of W2s or paid employees in each of 3 consecutive years
(2008-10 I believe).
What I haven't been able to figure out is why it chose the two staff
members it did to query me about. I am assuming it must be randomly
generated, as neither of these individuals is of Medicare age.
As far as the url goes, you need to make sure you have an additional
backslash after the word 'Match' and then it will work. I ran into
this too as I think the additional backslash was on the following line
in the letter and I didn't see it at first. You are also missing the
'hhs' in the url which I believe was in the letter.
