Good morning lovely librarians!

Does anyone have a card or form they use for patrons to fill out with their reference questions?
We have a few patrons who recently had lengthy requests that required a lot of time and it would be wonderfully awesome if they could write down those requests for us and we could get back to them.
I would rather not reinvent the wheel if it is not necessary. 😉
