OK, I checked starting with 1989-90.
 
Nobody's done it since then, so only the four teams I listed in the
first post have achieved this dubious mark of 20 wins and 20 losses.
 
Two more teams did come close:
 
 
Minnesota-Duluth, 1989-90, 20-19-1
Bowling Green, 1992-93, 19-21-1
