There are some answers in the interpretations at the back of the rulebook.
The discussion of kneeing includes this approved ruling:

RULING: A minor penalty

A similar entry is made for elbowing.

Under hitting from behind, the only AR is a scenario where an injury is
caused, and it is said that the referee assesses a DQ.

So it would appear to me that causing injury seems to mandate
disqualification, and is therefore the effective dividing line between a
disqualification and a misconduct.
