Paraphrased from '96 NCAA hockey rulebook:
loss of helmet or facemask:  immediately replace or go to bench.
  failure - minor penalty
  for goalkeeper loss - immediate whistle
mouthguard - initial violation, warning
  second offense in same game - misconduct (I assume 10 minutes)
  third offense, same player, same game - game misconduct
broken stick - immdiately drop (except goalkeeper)
  infraction - minor penalty
