You can find an "official" application form there.
 
Also, while corresponding with the Arrowhead Ticket Office when ordering my
tickets, the ticket agent said that "the last word we got from the NCAA"
was that credit cards would not be charged until Sept/Oct.  Things can
always change, but that was the word that I received (this was in April).
