As I've said before, seeding is really up to the "mood" of the current
selection committee. They can do whatever they want, including something
their predecessors have never done before.
 
So, to answer the questions:
 
No precedent.
No specific restriction, but the committee may tell you ahead of time
  what they will/will not do.
No, it's never been so lopsided.
 
There's only one way to find out: ask the committee members.
