Hello all.   

We have formed a search committee to explore options to expand our library foot-print.  Our small library has become too small for our future goals and objectives, as we continue into the 21st century.  One of our first steps is to develop a questionnaire to send to patrons, to determine their needs, desires for library services.   We're looking at a postcard format or brochure format so it will hopefully stand-out from other mail.   

We are looking for your recommendations in the following:  

  * sample questions to put in the questionnaire
  * We are also looking for ideas as far as format that would be the most effective for us in reaching patrons
  * Any other advice you may have to achieve our objective for patron input
