ERATE FORM 471 DEADLINE:  Form 471 window closes at 11:59 pm March 20, 2012.  

JUST A REMINDER OF THE REQUIREMENTS THAT WERE NEW AS OF 2011:

·      Item 21 attachments (description of service request) must be submitted during the 471 filing window)

·      You must include your FCC registration #.   FCC Registration number lookup: https://fjallfoss.fcc.gov/coresWeb/publicHome.do

·      If you are using a paid consultant, you must include consultant information including the consultant number

·      Although Technology Plans are still required by Maine State Statute, please be sure to check the correct box in Form 471, Block 6 # 26.     For Priority 2:  Check box that certifies you have an approved Technology Plan.  For Priority 1: Check box to certify to no plan require for E-rate.
