E-rate Form Deadlines

Although the deadlines aren't until October now is a good time to do two
required e-rate forms.  One form is for 2010 and one form is for 2009.

 

Form 486 - Deadline is October 29, 2010

For 2010 (For services that began July 1, 2010 and will end on June 30,
2011)

*	You must have already received your Funding Commitment Decision
Letter (FCDL) to fill out this form. (This will be pink.)
*	All the necessary information can be found on your FCDL.
*	Online Form 486 can be found at:
http://www.sl.universalservice.org/menu.asp

 

Helpful hints

*	Skip Block 2 - it does not have to be filled out after July 31.

For telephone applications

*	Block 4, # 8 - for telephone service, select Other and type in
None.
*	Block 4, #11 - for telephone service, select "c" - CIPA does not
apply

For Smartphone applications

*	Block 4, # 8 - for Smartphone (iPhone/Blackberry) service with
Internet, select Maine State Library
*	Block 4, #11 - for telephone service, select "a" - CIPA does
apply since the Smartphone uses the Internet.

 

Form 472 (BEAR) - Deadline is October 28, 2010

For 2009 (for services that began on July 1, 2009 and just ended June
30, 2010)

*	Make sure that you have paid all bills.
*	Collect all bills and add up the 12 months of bills for the
service dates noted above.
*	Check bills to make sure that all charges are eligible.  (Taxes
and USF fees are eligible but special services that go beyond a
telecommunications service, such as monitoring services for 911, E911,
or an alarm telephone line.
*	Find the 2009 Funding Commitment Decision Letter.  (This will be
yellow.) You need the Form 471 application number, the Funding request
Number (FRN) and the total amount paid.
*	PIN to login
*	Online Form 472 can be found at:
http://www.sl.universalservice.org/menu.asp under Utilities.

 

Helpful Hints:

*	Only fill in Column 12 - Leave 13 blank.
*	Make sure the service start date is 07/01/2009 (if you put 2010
it is a real problem and you will be denied)
