Scope: LDAP servers on NAME[A,D,I,F,K,O,P]

Summary: 
	Adding an index to the LDAP database.  We will make sure that NameO and NameP are not down at the same time and will not be down at the same time as AIF or K so systems properly configured with fail over will not be disrupted.  M has already been done.
