Dear Harried E-Rate Applicants,  This is a final heads-up reminder to help
you successfully complete your Year 2005 471s.

1.  The window has been extended through Friday, February 18th.  Please
remember that you must have your application filed by mid-day on Friday so
that you can print your certification pages, get them signed and in the mail
before the post-office closes.  All 471 documentation must be postmarked
before the end of the day Friday.  And please, please, please make sure you
send your certifications as certified or return receipt requested mail so
that you will have documentation proving that you sent them.

2.  Some of you continue to be listed on the SLD page as not having mailed
your certifications for 470s filed earlier in the filing season.  Please
check and make sure you have mailed those earlier certifications because
they also have to be mailed by the close of the day on Friday, Feb. 18th.
If you have a certified mail or return receipt postcard showing that your
documents were received you should be fine.  If you don't, it is in your
best interest to send another copy adhering to my please, please, please
message above.

3.  We've discovered a potential problem with the date listed on the ATM
template sent out earlier this year.  We are not sure how this will affect
ATM applicants.  If you've already submitted your 471 for ATM but want to be
safe, you can do a new 471 and use the correction, which is that the
contract award date should be 12/12/2000 instead of 12/01/2000 as listed on
the template.  You should also be able to correct the date when you receive
your 471 RAL notice.

4.  Please also remember that although they weren't required on your 471
that SLD's PIA people will be contacting you for FCC RN#s.  You should have
an FCC RN# for all entities represented in your application not just your
Billed Entity.    Here is the FCC site where you can apply for additional

those numbers so you should expect a request for them shortly.
