As to the first -- I don't think college ever had a two line pass.  Heck the
red-line icing did not get universal acceptance until around 1980 (after a
few years being a West-only innovation).

As to the second question -- You must be on-side at the blue line to receive
any pass from a teammate, regardless of where it came from.  Thus if I took
a pass from the goalie, I would be onside as long as the puck beat me across
the attacking blue line.
